Offline retrieval recall says the supporting document is in the top five. Production answers remain wrong, and adding more retrieved chunks sometimes makes them worse.
Design an oracle ladder that isolates retrieval, reranking, context construction, and generation. Name the intervention at each layer and the metric that prevents one component from hiding another component's failure.
